Account Application Form
The account application form is required to use a Temporary Account.
- When the temporary account is applied, Associate University Members are required to submit necessary certifications to apply for a temporary account.
- Submit the appropriate certifications that can prove your qualifications for using the system, along with an account application form, through the Responsible Person on Campus.
